Job Description: JOB SUMMARY:, The primary role of the Birth to Three Service Coordinator is to provide support, information and resources to assist families in dealing with needs and concerns related to their child’s development. This role is also responsible for providing service coordination and regular communication with families., DUTIES AND RESPONSIBILITIES:, 1. Responsible for all intake procedures to include obtaining current medical, social, psychological or other pertinent information from available sources regarding children referred to the program and providing information to parents regarding entitlements provided by the Birth to Three Program., 2. In conjunction with program therapist and families, schedule and participate in evaluation staffings, six-month reviews and exit staffings. May also participate in school districts multidisciplinary teams or other parent conferences as requested by other community resources or the family., 3. Develop and maintain good working relationships with other agencies, community resources, and work closely with county Birth to Three service coordinators., 4. Responsible for the coordination, facilitation, and review of the family’s IFSP., 5. Participate in community education that focuses on services for children and families. 6. Schedule home visits to reinforce continuation of programming in the home and provide support services.

Education Required: Bachelor or associate degree in human service related field or equivalent experience.
Experience Required: 1. Knowledge of Birth-to-Three Program and familiarity with community resources. 2. Case management and counseling experience in the field of developmental disabilities., 3. CPR certification preferred., 4. Basic computer experience (Microsoft Office, Excel, internet)., 5. Valid Wisconsin driver’s license and evidence of automobile insurance., 6. Must be willing and able to “bring caring to life”.
